How To Apply

Download the application form and print out all pages.

 (Note, DO NOT USE a standard Form 31, it must be the GLE2009 version)

 Make sure to read the information regarding arrival times and policies.

 

Fill out the "SAMT - Battle Stations Permission / Military Support Authorization (MSA)" form to insure that you have base access for the event  and (if applicable) your family and friends can attend graduation.

 

Complete the application as instructed and make sure you include all required information.

Any questions regarding requested and required information should be addressed to the Encampment Commander.   

 

A valild email address is required

It can be yours, your parent or guardian's, or a friend's.  If you would like to include multiple email addresses, feel free to indicate them on the form.

 

Contact telephone numbers and correct medical information are very important for your safety.  All information will be kept confidential.

 

Obtain required signatures.

 

Attach photo and appropriate check.

Make checks payable to: ILLINOIS WING SPRING ENCAMPMENT

          Please write "GLE2009" in the memo area.

If the cadet’s name is different from the account holder’s, please write their name in the memo field as well.

You will receive an attendance confirmation and Flight assignment in late March or early April.
